Terms of Service

1. Payment

Please refer to our most recent schedule of fees for an outline of the services that we offer and the associated prices.
The patient agrees to pay the fee in full at the time of the consultation. The patient agrees to pay all invoices on presentation. The patient agrees to pay all cost of collection, including all court costs and all and any legal/solicitors fees. Terms and conditions of treatment, credit and payment terms may be cancelled or changed by Maximize Health at any time without notice. All transactions are governed by the terms of Maximize Health documents. Where the patient is entitled to be indemnified by a 3rd party, Work Cover or Private Health insurer where a gap has occurred, the patient is responsible for the gap payment.
Maximize Health accepts cash, EFTPOS, credit cards (including American Express) and offers electronic health rebates (HICAPS) for the patients convenience. In the event of an electronic network failure, full payment is still required on the day of consultation. Payments of accounts can be done via the telephone or in person at Maximize Health.

2. Cancellations

  1. a. Cancellation Policy

    In our desire to be effective and fair to our clients and out of consideration to our professionals time, as of the 1st of August 2014 our Cancellation Policy will be enforced. Here at Maximize, we understand that cancellations might be based on unavoidable circumstances. Our professionals reserve their time for your appointment; therefore we ask that the client gives 8 hours’ notice of cancellation. If the client cancels with less than 8 hours’ notice or does not appear for a scheduled appointment without notifying Maximize, we reserve the right to charge a $50.00 cancellation fee. If the client cancels on a gift certificate, the certificate becomes null and void. Any Workcover, Veterans Affairs or EPC clients will be required to pay the bill privately. When you do not attend or do not cancel appointments with 8 hours’ notice, we are unable to fill that vacancy or offer these times to the clients on our waitlist. We appreciate your understanding and co-operation.

  2. b. Late and Missed Appointments

    If the patient arrives late to an appointment, but within appointed time slot, the patient will receive a shorter consultation in an effort not to inconvenience other patients. If the patient arrives after or does not attend the appointment, the patient will be charged the Cancellation Fee.

3. Consent to Treatment

The patient gives consent to our therapists to perform all necessary examinations, manipulations, therapy, rehabilitation and medical diagnostic procedures in accordance with their professional training and understanding of the patient’s injury. The patient understands that they have an opportunity to discuss with the therapist the nature and purpose of the physiotherapy care before treatment is rendered.

4. Limit of Liability

Maximize Health shall not be liable for, nor shall they accept any responsibility for any injury, sustained by any person or persons arising out of any treatments delivered at the place of business.
